Attention Parents/Guardians,
If your child is absent for any reason upon return please email this form to your class teacher or ask the teacher at the door for a hard copy to fill in at the classroom door.
If your child is sent home from school sick they must be 48 hours symptom free before they can return.
Children will not be allowed back into the classroom unless this form is returned.
